This helps the referrer, the patient, their family and friends understand how, with time and appropriate care, the patient will progress through the required levels of security and ideally back into the community.

We believe that consistent care with the same team of healthcare professionals and familiar services can be crucial to a patient's recovery. Therefore, clinical teams lead care pathway planning guided by clinical governance. This planning process is integrated fully into the CPA reviews.
