What a Partnerships in Care (PiC) hospital can offer you
Every PiC hospital is patient focused; this
means that our services are tailored to meet your needs
and expectations.
Your care
To ensure care is consistent during your time with us, you will
have your own care team.
Your rehabilitation will begin with a comprehensive assessment
of your needs and your care team will work with you to develop and
manage a treatment or recovery plan.
With your care team you will also set out what activities you
will be involved with each week; aiming for at least 25 hours of
activity a week that meet your own personal needs. This will
include therapies such as psychology and occupational therapy,
fitness, education, and once you are at a stage to do so, we will
encourage you to take up work placements in the local
community.
Our shared aim is for you to realise your potential. Your
future care pathway is considered from the outset and together we
will work hard to ensure your length of stay in our hospitals is
kept to a minimum.
"We have a weekly meeting on the unit where we have our
chance to say how to improve our environment."
"Patients really do move on from Oaktree Manor and make our
future look brighter."
"Hazelwood House is a place where we get to be motivated and
encouraged to help get us back into the community."
"Thanks to Elm Park I have got my family back."
Your needs
You will have access to your own ‘advocate’ – this is an
independent person, not part of the care team, who is responsible
for looking after your needs. They will help you should you need to
raise any concerns or problems you are having.
Your involvement
Some patients like to be active in their hospital’s ‘patient
involvement group’ (or patient council) where patients can discuss
a variety of topics that can be fed back to the hospital staff.
There will be other opportunities for involvement, namely through
our focus on the ‘Recovery approach’ to care which encourages
empowerment and involvement.

should be helpful if you have more questions on what it is like to
stay at a PiC hospital.
